<h2 id="aim">Aim</h2><p>The study evaluated effectiveness of metformin across renal function states in Diabetes.</p><h2 id="method">Method</h2><p>Retrospective, multicenter, observational study</p><p>Included patients: 1994 individuals withT2DMwithout prior DN obtained from three tertiary hospitals' databases.</p><h3>Treatment</h3><p>Metformin Cohort: Metformin and with at least one additional prescription.</p><p>Control cohort: Oral hypoglycemic agents other than metformin.</p><h3>Primary outcome</h3><p>Events of diabetic nephropathy (DN), net major adverse cardiovascular events (MACE) or in-hospital death and a composite of major adverse kidney events (MAKE) or in-hospital death.</p><p id="results">Results</p><p>Amongst patients without DN at T2D diagnosis, the continuous use of metformin vs other oral hypoglycemic agents was associated with a lower risk for:</p><ul><li>Overt DN (incidence rate ratio [IRRI, 0.82),</li><li>MACEs (IRR, O. 76), and</li><li>MAKEs(lRR, 0.45).</li></ul><p style="text-align:center;"><img src="https://cdn.megawecare.com/GHBY/Internal-Images/forest-plots-092341.png" width="876" height="500"></p><p>Compared with non-metformin or discontinued metformin use, the continuous use of metformin was associated with a low risk for MACE across CKD stages 3A ORR, 0.70), 3B ORR, 0.83), and 4 ORR, 0.71).</p><p>Similarly, the risk for MAKE was lower among continuous metformin users than in nonusers or discontinuous metformin use across CKD stage 3A (IRR, 0.39), 3B (IRR, 0.44), and 4 (IRR, 0.45).</p><h2 id="conclusion">Conclusion</h2><ul><li>Study findings suggest that administering metformin in patients with T2DM is associated with a lower risk of overt diabetic nephropathy and major cardiovascular events.</li><li>Furthermore, for patients with T2DM who have reduced renal functions, continuing metformin treatment is associated with lower risks of major kidney and cardiovascular events.</li></ul><p><strong>Metformin use in T2DM patients across various renal functions consistently correlates with a decreased risk of overt DN, MACE, and MAKE.</strong></p>
